Tony is 19 years old as of 2012. That's the year the Battle of New York happened, because I said so.
Howard Stark was alive until the Afghanistan demonstration (2010). Maria Stark and Edwin Jarvis died in a car accident when Tony was 15.
He has the arc reactor, and with it a decreased lung capacity and chronic pain issues, which he hasn't actually told anyone about.
Obadiah Stane died pretty much the same way, though he thought it was someone else in the suit that he fought against.
Pepper Potts was Howard's PA and Tony appointed her as CEO through a whole lot of legal bullshit, I don't fucking know, just go with it. She's the same age as she is in regular MCU canon.
James Rhodes was still Stark Industries military liaison. He thinks of Tony as a dumb little brother. Tony still builds him the War Machine armor to appease the military.
The palladium poisoning happens with a little less public spiral of destruction, and the film reel from his father hits a little harder. Fun teen emotions about bad dads.
The only people who know Iron Man is Tony Stark and not just some random bodyguard are: Pepper Potts, James Rhodes, Nick Fury, and Phil Coulson.
Natasha Romanoff does not know; she was sent to spy on Tony to judge whether he was mature enough to play consultant for SHIELD, nothing else.
